The role:

To assist in the provision of person-centered care, support and work as part of a team to achieve required standards. To provide personalised home support services to Service Users living in the community. To ensure Service Users retain their dignity, individuality and live the life they choose.

The Candidate:

  • Ensure that you wear full uniform with no adoptions and that the correct PPE is used at the correct times for the required tasks.
  • Attend support meetings/sessions such as team meetings, supervisions and appraisals when you are invited.
  • To report to the Manager any staffing problems
  • That gives you cause for concern about their ability to work or performance.
  • Take all possible steps to ensure the safety and welfare of the staff at all times.
  • Ensure that relatives wishing to pay fees or discuss the care of a Service user are directed to the Manager.
  • Ensure that all relevant information and instructions are reported to the Manager and recorded in the care plan.
  • Complete induction training and ensure all mandatory refresher training is completed.

Key Responsibilities and Accountabilities:

  • Always ensure the care / support delivered is done so in line with the service users care plan.
  • Ensure the highest possible levels of care and support are maintained by supporting/assisting Service Users, when required, with personal grooming (shaving, washing hair, styling hair, applying makeup) washing / showering / bathing, toileting, dressing, undressing, and all other aspects of daily living.
  • Support with prompting / administering the correct medication at the correct time. Collecting prescriptions and medication is also required.
  • Assist Service Users in all aspects of their care needs (e.g. physical, emotional and spiritual).
  • Provide attention when needed, whilst ensuring Service Users retain their comfort and dignity.
  • Pay particular attention to assisting Service Users who have limited mobility ensuring equipment / aids the service user is assessed as requiring, are used.
  • Pay particular attention to assisting Service Users who have learning difficulties, ensuring the use of aids provided.
  • Closely monitor Service Users who may be confused and / or who may present with behaviors that challenge.
  • Observe care planning needs for Service Users and complete written daily records as instructed and in line with the organisational policies and procedures.
