Salary: £15.40 per hour

Job Type: 35 hours per week

The Charity’s Community Services are provided in the Almshouse accommodation on site at Blackheath and at Ralph Perring Court in Beckenham, through the vibrant communal space in the John Morden Centre and the case management approach that proactively identifies, assesses, and supports the most vulnerable in our community.

Job Summary/Purpose:

As a Dementia care leader, your purpose will be to provide professional leadership and innovation within dementia care, building the capacity, confidence and competence to enable the team to respond effectively to residents living with dementia.
You will ensure residents with a “good life” by promoting dignity, independence, choice and fulfilment within a policy of managed risk taking. You will provide a person cantered care to residents by assessing, monitoring and evaluating individual needs as well as providing comfort, security and making a difference to their daily lives.
You will be responsible for the operational management of the dementia household, taking the lead for quality assurance and improvement in all areas, including medication management, electronic care records, assessment documents, incidents, accidents and near misses.
It’s important that you not only promote mental health and physical wellbeing to residents aiming to maximise their independence and meaningful engagement but also that you are able to update and maintain accurate care plans, records, assessments and are able to communicate this with families, GP’s and other professionals.

Key Duties & Responsibilities include but are not limited to:

  • As a Registered Nurse you will comply with standards of professional practice/ conduct and maintain valid registration with the NMC to practice.
  • Be accountable for the provision and co-ordination of person-centred nursing care through strong clinical leadership, role modelling and by ensuring good communication between multi-disciplinary teams, residents, and families.
  • Ensure that all staff adopt and maintain a person-centred approach to the development and delivery of high-quality clinical services and uphold the principle that each individual is at the centre of our services and the support they require.
  • Facilitate excellent partnership working within Cullum Welch Court and the Clover General Practice being supportive and inclusive and encouraging a culture of mutual respect, observing staff practice to enhance performance and always ensure a person-centred approach to care.
  • Work in collaboration with the Registered Manager ensure the efficient use of staffing resources in addition to Strengthening Morden College’s partnerships with Colleges of Higher Education to support the placements of Return to Practice, Student Nurse, and Paramedic practice placement processes, ensuring a working knowledge of current programme requirements, practice assessment strategies.
  • Oversee the timely completion of robust electronic care documentation, for individuals with nursing care needs, ensuring a personalised plan of care is created, in consultation with the individual and their family/representatives. Implement regular monitoring, evaluation and updating of care plans, ensuring all nursing care is appropriate for the individual and their wellbeing.

The Candidate:

  • You must be a qualified nurse and hold NMC registration.
  • You must have previous experience in a Senior Nurse or Clinical Lead role.
  • Has a minimum of 5 years hands-on nursing experience.
  • Caring and compassionate with an excellent clinical track record.
  • An inclusive and supportive management style balanced with strong leadership is a critical factor in the success of this role.
  • Has vision, is authentically warm, genuine, trustworthy, honest, reliable, consistent, and caring in all their actions and supports this culture within the organisation.
  • A well-developed sense of their own behaviours and how they impact on others
  • IT literacy specifically Microsoft packages
